myCooper Terms and Conditions of Use

This web site and any mobile version of this web site (individually and collectively referred herein as "Site" and "myCooper") and related services and content are provided by The Cooper Health System ("Cooper") to its patients. By accessing myCooper you agree to be bound by all the terms and conditions of this Agreement and by the myCooper Privacy Statement as posted. Cooper may modify this Agreement and the Privacy Statement at any time and such modifications shall become effective upon posting. 

myCooper Features

Electronic Messaging: Cooper will use best efforts to provide timely response to electronic inquiries made via myCooper. In some cases, the clinician who needs to respond to your inquiry may not be readily available. Our goal is to respond within 3 business days. Emergent requests should not be submitted via myCooper.

Secure Communications: All communications within the myCooper system occur over a secure connection. When new information is available on your myCooper account, you will receive an e-mail message to the email address which you provided to Cooper at your physician's office.

Sign On: When you enroll in myCooper, you will be provided with an access code, which will allow you to create a personal login ("myCooper ID") and a password. 

User Responsibilities

  1. By enrolling in myCooper, you elect to communicate via myCooper, you must supply Cooper with a valid email address in which you can be contacted. In the event your email address should change or your email service is somehow disabled, it is your responsibility to provide Cooper with an updated email address or terminate your enrollment to myCooper.
  2. You agree it is your responsibility to check myCooper for updated information as Cooper will communicate results, responses, etc. to you.
  3. When you are enrolled in myCooper, Cooper will use your myCooper account to relay non-actionable items, defined as those instances in which communications or test results do not require immediate action.
  4. Cooper will use telephone communications when relaying actionable items, defined as those instances in which communications or test results require discussion or follow up action on your part.

Security and Confidentiality: Cooper affords the same degree of confidentiality to health information stored on myCooper with in any other medium. Cooper has taken steps to make all information received from online visitors as secure as possible against unauthorized access and use. It is your responsibility to keep your login and password confidential.

Accuracy and Integrity of InformationCooper makes no guarantees whatsoever as to the correctness or accuracy of the Site. Please note that your records on this site may be limited to immunization records, Medications and Problem Lists, and accordingly the information on myCooper does not represent a complete account of your medical record. If you require a complete copy of your records for treatment or other purposes, please submit a Request for Records either to your physician practice or to The Cooper Health System Health Information Management Department at 1 Cooper Plaza, Camden NJ 08103.

Links to Other Sites: Cooper makes no representations whatsoever about any other website you may have access to through this site. Cooper does not endorse or accept any responsibility for the content or use of any other linked site. If you decide to access any third party sites linked to this Site, you do so entirely at your own risk.

Termination of Access: Cooper may restrict or terminate your access to myCooper at any time. 

Restrictions on Use and Ownership: Nothing contained on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of Cooper. All communication through myCooper may become part of your medical record and will remain the property of Cooper.

Availability: At this time, minor's myCooper contents are limited to vaccination history.

Proxy Access: Individuals who wish to request proxy access to another individual's myCooper account may do so by completing the Proxy Access portion of the myCooper Consent Form and submitting it to your Cooper practice. In cases where proxy access is requested to an adult's account, both requestor and patient will be required to provide photo identification and sign the Proxy Access portion of the consent form.

Miscellaneous: myCooper is offered as a convenience to Cooper patients and is provided "as-is" without any warranty of any kind.  The information contained in myCooper is not a replacement for proper medical attention. Access to myCooper is granted to users for the purpose of furthering their medical care. Use of the system for any other purpose may result in the suspension or termination of access or privileges. This agreement shall be governed by the laws of the State of New Jersey and any dispute hereunder shall be venued in, and the parties expressly consent to the personal jurisdiction of the appropriate state or federal court located in Camden County, New Jersey.

Discontinuing myCooper Access: If at any time you want to terminate your access, you may do so by calling the Cooper office you were given access to myCooper. In the event Cooper discontinues your access for any reason, you will be notified.
